Instructions for Application
To receive a tax credit for installing a renewable energy system, a taxpayer must submit an application to the Office of Energy Development (OED) for certification of a system's eligibility and the amount of any tax credit that may be claimed. When a tax credit has been certified, the taxpayer will receive a completed Utah State Tax Commission Form TC-40E that the taxpayer may use to document a tax credit claimed on his or her tax return. Credit may not be claimed unless the taxpayer has received a Form TC-40E from OED.
What Must Be Submitted for Certification of Tax Credits?
To be considered for certification, taxpayers must submit the appropriate forms and other required documentation. All applicants must complete and submit Form A: Basic Application Information. Each applicant must also complete and submit the form or forms appropriate to the type of renewable energy system for which credit is being claimed. (If the system uses more than one technology, forms should be completed for each technology in the system.) These forms are listed below.
Each of the forms below may be downloaded and opened in Adobe Reader or Adobe Acrobat. Note that each form has fields for entering information. You may enter the appropriate information directly into the pdf form that you download and then print the document for submission to OED. (Note that fields requiring a signature may not be completed online but must be signed on the printed copy.)

| Form PS: Passive Solar Systems | Required only for systems that use the heat of the sun and structural components of a building for heating or cooling |
| Form PV: Solar Photovoltaic Systems | Required only for systems using solar energy to produce electricity |
| Form ST: Solar Thermal Systems | Required only for systems using the heat of the sun to heat water or a building using mechanical systems |
| Form W: Wind Energy Systems | Required only for systems under 660 kW capacity that use the wind to generate electricity or mechanical power |
